There are many smaller insurance plans that fall under the umbrella of one of the above plans. If you are unsure if we take your plan, please send us an email and we can let you know.
At this time, we do NOT accept any Medicare or Medicaid/Medical Assistance plans including managed care plans or advantage plans. We hope to be able to offer services to Medicare and Medicaid beneficiaries in the future, but the process takes time and in some cases money which is not currently feasible for a small practice like ours. In most cases, Medicare and Medicaid beneficiaries are not able to pay out of pocket for services.
Self Pay: We do offer self pay services to those who do not have or do not want to use their health insurance benefits.
Fees: These depend on the services being provided. We are happy to discuss the fees with you to help you understand what your costs will be for services.
No Show/Late Cancel Fees: There is a $75 fee for appointments canceled with less than 24 hours notice or if you do not show up for your appointment. This will be charged automatically to your credit card on file.
Payments: It is expected that you will pay your co-pay or co-insurance amount on the date of service. We ask that all patients keep a credit card on file so that we can automatically deduct this payment. It is possible that you will have additional amounts due that we were unaware of on the date of service, this is usually related to your insurance deductible. If there is a balance due after insurance pays for your visit, your credit card on file will be charged for that balance. This is not “balance billing.” Balance due would be considered the patient responsibility noted on your Explanation of Benefits from your insurance company. We care arrange other payment methods if you are unable to have a credit card on file for some reason, but the preferred payment method is credit card on file. The card on file can be changed. Please let us know if you need to make changes.
Superbill: A superbill can be provided upon request for anyone seeking to try to obtain reimbursement from your insurance company for out of network benefits.
Good Faith Estimate: Under a new federal law, health care providers need to provide patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services. You have the right to receive a “Good Faith Estimate” for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment, and hospital fees. If required, a Good Faith Estimate will be provided in writing prior to your scheduled appointment. You can also ask for a Good Faith Estimate prior to scheduling an appointment.
Make sure to save a copy or picture of your Good Faith Estimate.

Billing Partners: Some clients are billed through our billing partners at Alma or Grow Therapy. If this situation applies to you, we will discuss this with you and make you aware of anything special you should know about this arrangement.
